Have a look at another patient of mine with an extraordinary donorarea. I will be posting graft details of his case below together with a timeline from before, 8 days and 12 months after. Feedback is welcomed. Unfortunately I don´t have pictures right after the surgery but I will upload photos the patient sent us on whatsapp the following days post op. 

 

Yes, I am fully aware that the patient was very young at the day of his surgery and usually crown surgery should not be done in these cases. However I did my calculations on his donorarea, and his donorarea could supply his whole head with more than 11 000 grafts with 3+ hairs/graft if needed later. He is aware that he must use finasteride/minoksidil throughout his life and he did so for a long time before his surgery. We went with a conservative classic hairline that suits his forehead structure and that will also look awesome on him when he is 70 years old.

 

Treatment details:

24 years old male with Norwood 4/5 who I started on finasteride 1.25 mg 5 times weekly and topical minoxidil a year pre op to stabilise the hairloss. Reconstruction of his hairline back to crown, 1 day procedure treatment. Extremely dense donorarea. 

Equipment used: Classic WAW FUE system with total of two new 0.85 Tornado hybrid punches. Saphire blade size 1.4 for the bulk part and metal blade size 1.0 and 0.8 for the last 1 cm of the hairline/feathered zone. 

 

Hair/graft ratio breakdown

 

1 Hair grafts - 480(mostly baby thin singles created under microscope)

2 Hair grafts - 590 -> 1180 hairs

3 Hair grafts - 1137 -> 3411 hairs

4 Hair grafts - 1312 -> 5248 hairs 

5 Hair grafts - 313 -> 1565 hairs

Total: 3832 Grafts -  11884 Hairs

Transection rate in this case was 3 % including partial transections. 

Average: 3.1  hairs per graft. This patient´s donor area was exceptional as you can see on the photos from the 12th month follow up.

Characterized by further frontal hair loss and enlargement of vertex, but there is still a solid band of hair across the top (mid-scalp) separating front and vertex.
