Treatment details:
This patient came to me being concerned about his temples and receding hairline. I started him on Finasteride 1.25 mg 5 times weekly prior to his surgery that took place on the 22nd of april 2024. He has a great family history with all of his male ancestors looking like Elvis Presley when they were 60 years +old. There were 2 things here that were extra tricky though.
1) Patients with light skin and dark thick hair(latinos, asians, people from the middle east) have much more contrast between the skin and the hair and it is more difficult with these cases to make the hairlines look truly natural, soft and dense at the same time. Using the right amount of singles, especially peri-auricular baby thin singles are crucial to achieve a result where the patient does not look operated.
2) The easy thing here would be to smash in 1600-1800 grafts to be sure that the density on the thinning area would be equal to the rest of the head. The more difficult thing here was to minimise amount of grafts and ensure equal density on the whole head once his full result would appear. I always try to minimise amount of grafts, not to waste them in vain for the future if/when the hairloss progresses.
Equipment used: WAW DUO FUE system with total of one new 0.85 hybrid tornado punch. Metal 45 degree angled blade size 1.2 for the bulk part and metal 45 degree angled blades size 1.0 and 0.8 for the last 1 cm of the hairline/feathered zone.
Hair/graft ratio breakdown
1 Hair grafts - 411 (mostly baby thin singles from the peri-auricular area created under microscope but also some regular ´´thicker´´ singles from the regular donor area. The baby thin singles were strategically placed in the very front of the hairline and the regularly sized singles some millimeters deeper into the hairline)
2 Hair grafts - 220 -> 440 hairs
3 Hair grafts - 306 -> 918 hairs
4 Hair grafts - 248 -> 992 hairs
5 Hair grafts - 15 -> 75 hairs
Total: 1200 Grafts - 2836 Hairs
Transection rate in this case was 5 % including partial transections.
Average: 2.36 hairs per graft.

There is a slight recession of the hairline around the temples. This is also known as an adult or mature hairline.
