This case features a 48-year-old male patient with Norwood 6 hair loss involving the frontal area, mid-scalp, and crown. He had no previous hair transplant surgery and had not undergone prior medical treatment.
The patient’s main goal was to improve the overall appearance of hair loss from the frontal region through the crown. Given the extensive balding area, the surgical plan required careful donor evaluation, donor density assessment, and long-term planning before proceeding with a large single-session FUE procedure.
A total of 6,000 grafts were transplanted in one session using the FUE technique, with implantation performed using forceps. The grafts were distributed strategically across the frontal hairline, mid-scalp, and crown. Greater priority was given to rebuilding a natural frontal frame and improving mid-scalp density, while the crown was addressed with a more conservative coverage strategy to preserve donor reserves and maintain future planning options.
This case demonstrates the importance of donor-conscious planning in advanced Norwood hair loss, especially when performing a large single-session FUE restoration. Careful graft allocation, age-appropriate hairline design, balanced front-to-crown coverage, and preservation of future donor capacity are essential parts of the surgical strategy.
