I would like to present the case of a 39-year-old African American patient with no significant androgenetic alopecia but a high hairline. He initially went to a clinic in Atlanta where the entire surgery was carried out by technicians and there was no doctor in sight. The growth from this surgery was close to 0%.
When he presented to my clinic, he still wanted to achieve his goal of lowering his hairline. Since 2,500 grafts were already wasted from his donor area, donor mapping was performed to homogenize his donor area. We planned a two-stage procedure. The first procedure was done with only 800 grafts to evaluate the growth and make sure there were no patient factors resulting in poor growth. At the one-year mark, the patient demonstrated excellent growth. Following this, a second, larger procedure was performed using 2,000 grafts to lower the hairline and frame his face. The patient is now one month out from the procedure, and while we wait for the final results, he is already ecstatic with how things are looking.

The Norwood Class A patterns are characterized by a predominantly front to back progression of hair loss. These patterns lack the connecting bridge across the top of the scalp and generally have more limited hair loss in the crown, even when advanced.
