Hair Plugs And Hair Transplant Repair At Bernstein Medical

Hair plug procedures, popular in the 1960s and 1970s, often resulted in an unsightly "pluggy" or "dolls head" look. In the past we tried to work around the plugs with fine grafts in order to camouflage them, but this proved ineffective. At Bernstein Medical, we now find it best to totally remove the old plugs, put them under a microscope, dissect them into smaller follicular units and then re-implant them. This process is called Combined Hair Transplant Repair. In this way, many of the cosmetic defects created by hair plug procedures can be partially or completely reversed.

How Does the ARTAS Robotic Hair Transplant System Locate and Dissect Follicular Units?

Dr. Bernstein describes how Robotic FUE hair transplant procedures require a device called a tensioner to help the robot locate and extract follicular units from the scalp.

Specifically, in doing a robotic hair transplant, we use an instrument called a tensioner, a small window like device, to stretch the skin of the scalp; we place it on the scalp and then we expand it. The expansion puts traction on the skin to hold it firmly in place.

This tensioner also has little tiny dots around the edges that are coded for the robot to visualize. The robot scans the edges of the tensioner and incorporates it into the software.

The robot then uses a 3D optical system to visualize the follicular units; it then aligns the cutting part of the robot with the angle of the hair follicles, and it makes a small incision around the hair follicle.

Evolution of FUE Instrumentation: From Hand-held Punch to Robotic Extraction

Instruments used in FUE hair transplant procedures have evolved from the hand held punch tool to robotic-assisted follicular extraction (Robotic FUE). 


Surgical instruments for follicular unit extraction (the harvesting of follicular units in a hair transplant) began with the use of a simple hand-held punch. New hand-held tools were developed to improve the FUE process, these included a motorized two-step extraction instrument. However, because hand-held devices required consistent precision by the doctor to avoid damaging grafts they were difficult to use. The ARTAS System, a robotic hair transplant system, overcomes these difficulties by using its image-guided computer-driven system to extract grafts with precision and consistency. The result with the robotic system is a more efficient follicular unit extraction procedure.

Robotic Hair Transplant Procedure Part 2a: Manual Recipient Site Creation

In Part 2 of our three-video series on FUE hair transplantation, Dr. Bernstein describes the recipient site creation process in detail. He explains why, in FUE procedures, the recipient sites are created before the follicular unit grafts are extracted from the donor area. “Pre-making” recipient sites is a key improvement to the FUE procedure that Dr. Bernstein was the first to describe in medical literature. It is beneficial in several ways, the most important of which include reducing the overall time grafts are out of the body and creating a “fertile bed” for the follicular unit grafts. Both of these processes contribute to increased graft survival.

Robotic Hair Transplant Procedure Part 2b: Robotic Recipient Site Creation

Dr. Robert M. Bernstein and the team at Restoration Robotics, Inc., have used the Bernstein Medical – Center for Hair Restoration facility, in New York City, to demonstrate a test of the new recipient site creation capability of the ARTAS Robotic Hair Transplant System.


At the 2nd ARTAS User Group Meeting in February 2014, Dr. Bernstein introduced hardware and software technology by which the ARTAS Robotic System can create recipient sites in an FUE hair transplant. The addition of the recipient site creation capability to the robot, which until now was used solely to extract follicular units, brings the system closer to performing all critical aspects of a hair transplant procedure.

In using the updated ARTAS robot, the doctor takes photographs of the patient which are fed into the robot’s computer system. The ARTAS™ Hair Studio converts the photos into a 3-D model of the patient. This model can be manipulated and saved for use in the patient’s procedure. The physician then designs the recipient site incisions in order to create the most natural looking hair transplant. During the procedure, the robot uses its guidance technology to avoid existing hairs while creating up to 1,500 recipient sites per hour. The automation of recipient site creation improves a procedure that, with manual tools, can be physically demanding and prone to human error.

The final step of a robotic hair transplant will be the automated insertion of follicular units into recipient sites created by the ARTAS. This step is likely 2 or more years away from use in a hair restoration clinic.

Robotic Hair Transplant Procedure Part 3: Graft Harvesting

In Part 3 of our three-video series on FUE hair transplants, Dr. Bernstein narrates as he and his team of assistants prepares the patient for graft harvesting by the ARTAS Robot for FUE. This includes positioning an apparatus called a tensioner on the donor area, administering tumescent fluid to the dermis, programming the robot for the specific procedure and aligning the robotic arm over the donor area.


As the robotic extraction commences, Dr. Bernstein discusses the reason why the robot is tasked with extracting only 2-, 3- and 4-hair follicular units while avoiding the smallest 1-hair grouping. He also discusses the robot’s 2-step extraction procedure in which a 0.9mm sharp punch is used to score the skin and that is followed immediately by a 1.1-mm rotating blunt punch to dissect the deeper portion of the grafts from the surrounding skin.

Watch as Dr. Bernstein describes the actions of the robot as it uses its specially designed image-guided sensors to select the next graft, position itself to follow the angle of the hair, then use the 2-step technique to dissect out the follicular units.

After the robot performs the extraction, Dr. Bernstein’s team uses special forceps to remove the follicular units from the scalp and place them first in Ringer’s Lactate and then in chilled HyperThermosol-ATP a medium designed to prolong the life of the grafts after they have been removed from the scalp.

FUT Hair Transplant Procedure Part 1: Consultation

In Part 1 of our two-video series on Follicular Unit Transplantation (FUT), Dr. Bernstein is seen in consultation with a patient in preparation of his hair transplant procedure. In FUT, follicular unit grafts are harvested from the back and sides of the scalp through a long, thin donor strip. Dr. Bernstein reviews with the patient the target number of grafts to be harvested and how he arrived at that number. Next, he goes over the aesthetic plan for the transplant and how the patient will be able to groom his hair when the transplant has matured. Sedatives are administered and the patient’s vital signs and blood oxygen are recorded.

FUT Hair Transplant Procedure Part 2: Strip Harvesting

In Part 2 of our two-video series on FUT, Dr. Bernstein walks us through preparing the hair transplant patient’s donor area before harvesting the donor strip. The donor area is shaved and injections of anesthetic are administered. Dr. Bernstein then incises the donor area so he can remove the thin strip of skin containing the follicular units that form the basis of the hair transplant procedure. After removal, the strip is placed into a dish containing Ringer’s Lactate at room temperature before going to the dissecting table. From there, we see the painstaking process of dissecting out the individual follicular unit grafts for later placement into balding areas of the scalp. Dissected grafts are held in a special chilled biologic solution to maximize graft survival.

How is the ARTAS Robot Used During a Hair Transplant Procedure?

The follicular unit extraction (FUE) hair transplant procedure consists of four steps: 1) separation of follicular units from the surrounding tissue, 2) removal of follicular units from the scalp, 3) creation of recipient sites, and 4) placement of follicular units into these recipient sites. The first step is performed by the ARTAS System -- the part of the FUE hair transplant which requires the greatest precision and that is most subject to human error.


How Young Is Too Young To Have A Hair Transplant?

"I think that unfortunately in our society, hair is very important. I think that you can really change people's lives. "I think the most important thing in my practice is that we don't view people as coming in for hair transplants. We view them coming in as having a hair loss problem or self image problem. First of all, we try to understand exactly what bothers them. And then the next thing is to try to find out what the best solution is. Sometimes people just need a little hand holding. Many people come in having very early hair loss and need medical therapy. Many times women come in and they don't have androgenetic alopecia. They either have hair loss from thyroid disease or some other condition. So it's really important to evaluate the person very thoroughly first and make sure that those people who are eventually going to have surgery have considered other options and are really appropriate candidates for surgery. We look at evaluating people as having hair loss, not as being candidates for hair restoration surgery."

"I think that the internet has helped a great deal because it allows people to get information from other patients. "My staff sometimes teases me because my opening question is always 'What's up?' And I think it's very important because very often we're very surprised what people come into the office for. And I think what we think might be bothering a person might be totally different from what's actually bothering them. I think an open ended question initially really gets a sense what's in the patient's mind. Because that's what we're trying to help. Then the next thing that's important is to do a very thorough evaluation and that comes with a very extensive history. And then a very detailed examination. In our office, we measure density, scalp laxity, the hair counts, miniaturization so we really try to get a sense of what the hair reserves are. And then to find out exactly what their needs are and to put the two together. You have to make sure what they actually want is what we can actually do. And in many cases, they're very different at least initially. And then part of the consultation is to try to have them come together and try to get their expectations to match what we can actually do for them."

Although I don't think it made much of a difference, to him it was a big deal. So he came back one year after the hair transplant and said that sales have picked up, he's more confident. Many of the people that feel they've had the biggest change are those who've had transplants previously with the old techniques. We're able to take the old procedures where people had hair plugs pointig in the wrong direction and actually change it. And people who would walk around with hairpieces or baseball caps for years were finally able to be free of that. And then technology is only evolving over the last several years and we're finally able to reverse a bad hair transplant".

"I think that it's interesting because when we gave the first presentations in the society, the ISHRS, nobody was doing the procedure and people thought we were crazy. It wasn't until patients started to chat about the procedure on the internet that doctors really started taking notice. You needed microscopes, you needed a huge team and people just didn't want to do it. It wasn't until there was a popular demand for the procedure that doctors started taking notice. And it wasn't until about 1999 or 2000 that even the society was really even talking about it at meetings. They were still talking about different ways to make slit grafts and scalp reductions and all of the sudden around 2000, things started to turn around where the meeting now is almost entirely follicular units. So it's really been an incredible change. It's been exponential. It started very slowly the first couple of years."

"I did my dermatology residency at Albert Einstein and when I was chief resident, I did a bunch of hair transplants. Then I joined this staff in Columbia and for years I really saw a lot of people getting really bad results with hair transplants. Then in 1994, I went to the Toronto hair meeting and I heard that things had started to change. Bill Rassman was doing these large sessions with micro-grafts and I was really shocked because I thought the results were so good. So I met him after the meeting and he invited me out to his clinic in California and I was just really blown away. And then I had this idea of using follicular units..."

"It was surprising to me when I saw hair transplantation being performed. People were really looking past the fact that hair grew in these naturally occurring groups. People treated the donor strip as this kind of massive hair. And it seems so obvious now, but 10 or 11 years ago, nobody was considering to use these tiny little groups of naturally occurring hair. And we started doing the procedure this way and it was obviously much better. The grafts were very small, they had a much higher hair content and they were able, much more importantly be able to fit into very small recipient sites. And that's a very significant thing because when the sites are small, you can use thousands of them without injuring the scalp. That really moved the hair transplantation from these multiple small procedures to these large sessions where someone's hair restoration can be finished in one or two sessions..."

"I think initially, it was a battle, should I do it or not. Once people started to catch on then it was really sort of a paradigm shift. People weren't fighting the procedure anymore, they were thinking how they could do it better. Then you had the really good minds of the people in the field putting their energies towards improving the technique. Over the last 11 years, the major things have been the recipient sites getting much smaller and the microscopic dissection getting much better. Because the sites are smaller, the graft numbers are getting much bigger. I think that also, in the old days, the procedure took so long that many people never got to see their long term results. I think now that the procedures are done in 1 or 2 sessions, the time from their procedure to seeing the final result has been compressed..."
